Admission
Thank you for your interest in Southeast Works! In order to complete the referral process the following items must be done/completed:
- Complete the application and return it as soon as possible.
- Enclose copies of your insurance cards (Medicaid, Medicare, etc.), Non-Driver ID, Social Security card.
- A physical exam form completed by a licensed physician and returned to this Agency as soon as possible. You must also return a copy of your PPD test.
- Please enclose a copy of your full ISP to include all Habilitation and Behavior Support plans; copies of initial LCED and most current LCED.
- Please enclose a copy of a Psychological Assessment dated within 7 years.
- Please enclose a copy of the Notice of Decision (NOD).
- Please enclose copies of the following summaries if they are available
- Social Summary
- Educational Summary
- Vocational Summary
