Medicaid Service Coordinator needed to provide services to adults with developmental disabilities, Including enrollment of eligible program participants in Medicaid Waiver funded programs, development of Individual Service Plans, maintenance of required written documentation of required services, and the provision of services such as linkage, referral, crisis intervention, and short term counseling. Full Time.

Requirements:

MA degree in Human Services and 1 year experience working with people with developmental disabilities, or BA degree in Human Services and 2 years experience working with people with developmental disabilities. Candidate must be a Qualified Mental Retardation Professional (QMRP), and possess a valid New York State Driver's License
